What is YCC?
Youth Care Coordination (also referred to as mental health case management) is a service that supports children, adolescents, and transitional aged youth with serious mental health disorders with gaining access to needed medical, mental health, social, educational, and other services in Charles County, Maryland. The primary goal of of this program is to connect clients with needed services, including medical, social, educational, mental health, housing, insurance, employment, financial, and other services to support successful functioning in our communities.
Care Coordination is a collaborative process which empowers families to achieve positive outcomes through a structured, family-centered approach by providing support in the community and reducing the likelihood of out-of-home placement and referral to a higher level of care.
What Will We Do?
Youth Care Coordinators help families plan the services they need, link them to the community resources services, coordinate the services if needed, and monitor how the youth and families are responding to services. Additionally, skills training is provided to youth, along with their families and caregivers to help them manage their symptoms and behavior independently.
The Care Coordination program also includes emergency and crisis support and advocacy services. Our care coordinators help ensure that every youth in our program has the personalized services and supports they need to thrive.
Parent and guardian participation is strongly encouraged throughout the care coordination process. Care coordinators work with the entire family unit, not the youth alone. Active family involvement is essential to building and implementing an effective Plan of Care. Under Maryland law, youth age 12 and older may consent to mental health services independently if the provider determines they are mature and capable of giving informed consent.
Where will services take place?
This depends on you or your child's needs and the setting where those needs could best be met. Sometimes our visits will be one-on-one at your home, at the school, our office or other community setting.
Who is eligible?
Youth between the ages of 5 and 18 who reside in Charles County, have a qualifying psychiatric diagnosis, and are enrolled in Maryland Medicaid may access Youth Care Coordination services. Youth who enroll prior to age 18 may continue to receive services until age 21.
No Medicaid? You May Still Qualify.
Youth Care Coordination services are primarily funded through Maryland Medicaid. However, youth who have commercial insurance or no insurance at all may still be eligible for services through TCM Plus, a separate state-funded program administered by the Maryland Behavioral Health Administration.
TCM Plus is designed for youth with intensive mental health needs who do not have Maryland Medicaid. Referrals go directly to BHA and do not go through Carelon. Slots are available on a first-come, first-served basis. To find out if your child may qualify, contact us at 240-207-4513.
Who Can Make a Referral?
Youth Care Coordination accepts referrals from a broad range of sources. Any of the following may initiate a referral:
Parents and guardians, hospital discharge planners, school counselors and school-based staff, outpatient mental health providers, pediatricians and primary care providers, the Department of Social Services, foster care agencies, juvenile courts, community organizations, and the youth themselves.
